Dr. Philip Moons are a professor in nursing science at the Center for Health Services and Nursing Research of the KU Leuven – University of Leuven, Belgium. He is mainly involved in outcome research in congenital heart disease, and developed and implemented the role of advanced practice nurse in the Adult Congenital Heart Disease Program of the University Hospitals of Leuven. His research spearhead is ‘Transition and care’.

For his work, Philip Moons received Martha N. Hill New Investigators Award 2004 from the American Heart Association and the Atie Immink New Investigators Award 2008 from the European Society of Cardiology. He is a fellow of the American Heart Association (FAHA), the European Society of Cardiology (NFESC), and the European Academy of Nursing Science (FEANS).

The spearhead of Philip Moons' research is 'Transition and Care'. This research line comprises three themes. * Care for transition: Research on patients who are transitioning from one life phase, medical condition, or social role to another. * Transition of care: Research on patients in whom care is transferred from one setting to another. * Care in transition: Research on healthcare delivery models that are in transition, to be adapted for future needs of patients and populations.

The target populations are persons with chronic diseases, healthcare professionals, and policy makers.
